Anna Marie M. (Forgac) Chulik of Naples, Florida--formerly of Cortland, Ohio--died on October 22, 2022.
Anna Marie was born on March 16, 1934, in Youngstown, Ohio, daughter of John and Anna Forgac. She
graduated from Ursuline High School and St. Elizabeth Hospital School of Nursing. Mrs. Chulik received
her Bachelor of Arts degree from Youngstown State University and her Master’s in Education from Kent
State University.
Anna Marie had a notable career in Nursing Education. After working at St. Elizabeth Hospital, she was
an instructor at Choffin School of Practical Nursing. She then became the first Director of the Warren
City Schools of Practical Nursing in 1962. In 1970, she went on to be the Associate Director of the School
of Nursing at Trumbull Memorial Hospital in Warren, Ohio, and in 1978 was named its Director, and she
held that post until the school’s closing in 1988.
Anna Marie has been a member of: the Board of Trustees of the Eastern Ohio Health Education Network
(MSAHEN); the Warren Civic Music Association; the National League for Nursing; St. William Church in
Champion, Ohio; and St. Elizabeth Seton in Naples, Florida.
Mrs. Chulik married her husband Theodore on August 24, 1957. He passed away November 12, 2003.
She is survived by one nephew, James Forgac; three nieces: Joann (Forgac) Webber, Patricia (Forgac)
Bittinger, and Jane (Forgac) Bittinger; and many friends.
Besides her parents, Anna Marie is preceded in death by her sister, Sister Margaret Mary VSC; two
brothers: Father James Forgac OSB and Joseph Forgac; and nephew David Forgac.
Anna Marie will be remembered lovingly for her spunk, kindness, quick wit, many accomplishments and
an engaging grin.
A Memorial Service will be held Wednesday, November 2 at 11:00am at Fuller Funeral Home, 1625 Pine
Ridge Rd., Naples, with live streaming also available on the tribute page of FullerNaples.com website,
under Anna Marie Chulik’s name.
Burial Mass will be held at St. William Church in Champion, Ohio, on Wednesday, November 9, at
10:30am. Inurnment will be immediately afterward at Calvary Cemetery in Youngstown, Ohio.
